What should I wear to my first appointment?

Wear flexible, comfortable clothing (e.g., shorts, leggings, T-shirt). With your consent, we may assess sensation and muscle function during your evaluation—skin access helps ensure accuracy.

Please wear supportive, closed-toe shoes – like sneakers – to ensure safety during movement and balance activities.

Do you take insurance?

At Cortex Rehab, we operate outside most insurance networks (except Medicare) to deliver personalized, high-quality neurological care without restrictions. While some plans limit therapy options, we offer documentation for out-of-network reimbursement if eligible. Our priority is uncompromised care that empowers your recovery.

How can I find my out-of-network benefits?

Many patients don’t realize their insurance may reimburse part of their care—even out-of-network. Call your insurer to check benefits, and we’ll provide a superbill for claims. Need help? We’re here to guide you.

What is a superbill?

A superbill is an itemized receipt with all the details your insurer needs for out-of-network reimbursement—services, diagnosis and treatment codes, and payment info. While we don’t bill insurance directly (except Medicare), we’re happy to provide one for your claim.

Do you offer package pricing?

Yes! After your evaluation and treatment plan are complete, we’ll recommend a package that fits your goals—and helps you save money compared to paying per session.

What forms of payment do you accept?

We accept all major credit/debit cards and HSA/FSA for easy payment.

Do I need a referral to see you?

No referral is needed! Montana allows direct access to physical therapy, which means you can schedule an appointment without a physician’s referral. This helps you get started with care faster and more efficiently

How do I get to your clinic?

Cortex Rehab is located inside Access Fitness at 431 North 7th Avenue.
After parking, enter through the main entrance and take a right—our clinic is the first door on your right.
